To ensure your registration is smooth and stress-free please follow the simple steps below:
1. Read the HCA Patient Registration Guide
This guide clearly details the terms and conditions of admission for insured and self-funding patients, as well as containing information on the Data Protection Act 1998. Request a Patient Registration Guide if you haven't received one already.
2. Return your Registration Form
Please check that you have the correct paperwork ready. If you’ve been booked for an appointment in advance, some of your forms can be returned to us ahead of your appointment using the pre-paid envelope provided. Note, you cannot be seen in Chelsea Outpatient Centre until these forms are completed.
3. Double check your appointment time
Please ensure you arrive in good time for your appointment.
